Personalised support that fits your routine
From your initial call, our experienced team will take the time to understand your situation, answer your questions and explain how Excellent Care can support you. We will send you a detailed information pack and, if you choose to proceed, arrange a face-to-face meeting in the client’s home. This allows us to get to know the client and their family, assess their needs and discuss a personalised Care and Support Plan, including risk and medication assessments and a proposed schedule of visits. We can usually have a home-care package in place within seven to ten days.
We will then carefully match the client with carers who are well suited to their individual needs and personality, prioritising continuity by providing the same small team wherever possible. Before regular visits begin, the Care Manager will introduce the client, family and carers at the client’s home. After each visit, carers will update our care management platform so the family can stay informed, while the dedicated Care Manager provides ongoing support and regularly reviews the care plan. If the match is ever not quite right, we will work with you to find a suitable alternative.
